At DuBois Nursing Home, the Just Breathe Recovery Program offers care in a skilled setting. We have respiratory therapists who are on site five to seven days per week. A few additional benefits to name are: educating staff, collaborations among healthcare providers, working as a well-versed team, and leveraging existing strategies of medication stewardship. 

Medications required as part of each individual’s treatment plan, will be continued while under the care provided by DuBois Nursing Home and while participating in the Just Breathe Recovery Pulmonary Care and Recovery Support Program. Any changes made to the administered medications will be provided, along with an updated report with administration instructions for at-home maintenance and management. A medical report will also be forwarded to the program participants Primary Care Physician and Pulmonologist.

What is Respiratory Therapy? 

  • This specialized therapy involves the evaluation and diagnosis of individuals with respiratory issues.

DuBois Continuum of Care Community in conjunction with Dr. Bansal and his staff work together to provide Just Breathe Recovery Program participants with the necessary personalized pulmonary care and rehab so that each individual’s success will be supported through providing the specialized equipment and treatment plan that continues when discharged home. 

Setting up our program participants with At-Home follow-up and training, is key to continued success for better living and reaching attainable goals.

Just Breathe Recovery Program is a Pulmonary Recovery Support Program. 

DuBois Continuum of Care Community provides a results based treatment plan for individuals with acute conditions affecting their functional abilities with a primary and/or a secondary pulmonary diagnosis. 

The results from pulmonary specialty care and rehabilitation, such as Just Breathe Recovery, enhances functional independence and teaches self-management strategies, with the overall goal to prevent the need for re-occurring hospital visits or stays. From start to finish, your respiratory recovery goals become more attainable and your success story becomes more real. 

Our Team Consists of: A Board Certified Pulmonologist (Sandeep Bansal, MD, FCCP, FACP), Registered Respiratory Therapist, Specially trained staff in Pulmonary Disease, Attending Providers (Dawn Jeannerat, AGACNP-BC and Todd Sudik, CRNP). 

Program Features Include: 

  • Specific protocols (procedures) for the management of Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), Pneumonia, Emphysema, Asthma, and other respiratory diseases. 
  • Consultations by a Board Certified Pulmonologist and team of Nurse Practioners 
  • Evaluation, and treatment by a registered Respiratory Therapist 
  • Specially trained staff in pulmonary disease 
  • Short-term, and long-term oxygen needs education plus at-home training 
  • On-going patient, and staff education 
  • Pulmonary function testing, and functional performance assessments 
  • Follow-up after discharge to support your success at home
